Understand the two documents
Do not describe the statement in lieu as the embassy’s certification that you personally are single. Canada does not issue that individual certificate. Ask the Philippine receiving office whether it will accept the statement together with your supporting evidence. Record its answer before preparing the consular request.
Prepare the request carefully
Use the Manila mission’s current notarial-service instructions and its form. Its procedure calls for a statutory declaration signed before a notary and supporting identification or civil-status records. Check which copies must be certified. The mission’s instructions should determine how you sign, what you enclose and how you arrange return delivery.
Keep an application copy
Retain a copy of the declaration, the submitted inventory and delivery references. When the statement arrives, compare its details with your records. Present it through the process requested by your local registrar. The statement itself does not authorize a ceremony or replace the Philippine marriage license.
